Depends on what you want to do. If you just want to create a Today theme, there are a couple of programs that will create smartphone-sized themes. I use a verion of the Thememaker program (http://www.thememaker.biz/) called Thememaker McDeb (http://www.mcdeb.com/tmmcdeb/), which has a free demo if you want to try it out.

If you want to do more than create a Today theme, but actually change the menubars, screen layouts, everything, I'd highly suggest Lakeridge Software's WisbarAdvance 2 and Wisbar Advance Desktop (http://www.lakeridgesoftware.com/products.php?prod=WisBar+Advance). It's the number one most important piece of software on my Axim Pocket PC! There have also been lots of themes released lately that emulate the iPhone's style, so you might not even have to create your own. Here is one I found from a quick Google search: http://iphonego.org/?p=128.
